Our referral process:
If you wish to refer a patient to us, there are two options: via fax or phone. You can utilize fax requests by sending patient demographics, insurance information, and their most recent chart notes to 859-264-0447. We will make the appointment and contact your office as well as the patient with the appointment information. If you would prefer to call, please call our main line at 859-264-0445 and a scheduling specialist will assist in making the appointment.
How to prepare your patient for their appointment with us:
In order to prepare patients and help expedite their appointment with us, please provide them with a New Patient Packet for our office. This has all the paperwork they will need for their appointment as well as directions to the office and what to expect during their appointment. Alternatively, they can complete this paperwork on our portal at https://www.chkin.com/br3969/login. Please remind them that the appointment will be 2-3 hours and that they will be dilated.
